How the Role of the MR technologist has changed over the last 25 years

MRI is one of the most powerful diagnostic tools in medicine. While imaging technology has advanced rapidly over the last two decades, MRI safety has evolved just as dramatically.

Twenty-five years ago, MRI safety focused primarily on identifying obvious hazards and preventing catastrophic events. Today, MRI safety is an integrated system of governance, policies, standardized workflows, multidisciplinary collaboration, and increasingly complex clinical decision-making.

Perhaps no role has changed more than that of the MR technologist.

Today's MR technologists are no longer responsible solely for acquiring high-quality images. They are expected to evaluate implants, perform increasingly comprehensive patient screening, coordinate with physicians and physicists, document safety decisions, implement departmental policies, and help lead MRI safety programs.

The evolution of MRI safety is, in many ways, the evolution of the MR technologist.

One Tragic Event Changed the Course of MRI Safety

Every profession has defining moments.

For MRI safety, that moment came in July 2001 with the death of six-year-old Michael Colombini following a preventable MRI accident involving a ferromagnetic oxygen cylinder. Although MRI incidents had occurred before, this tragedy fundamentally changed how the imaging community approached safety.

In response, the American College of Radiology (ACR) convened its Blue Ribbon Panel, which published the first comprehensive MRI safety guidance in 2002.

For the first time, MRI safety was presented not simply as a collection of precautions, but as a coordinated system built on policies and procedures, education, standardized workflows, defined responsibilities, and continuous improvement.

Many of the principles established more than two decades ago continue to guide MRI practice today.

From Rules to Systems

The earliest ACR guidance dramatically expanded what MR personnel were expected to do beyond operating the scanner.

Technologists became responsible for supervising non-MR personnel, screening patients, visitors, and staff, removing ferromagnetic objects, verifying equipment safety, preventing thermal injuries, reporting safety events, and following standardized procedures within the MR environment.

These responsibilities existed in addition to everything required to safely perform an MRI examination itself.

MRI safety was no longer just about what happens once the patient is on the table. It became about the systems, processes, and clinical decisions that support safe patient care before, during, and after every examination.

As MRI Became More Complex, So Did Safety

The years that followed brought rapid changes to MRI technology.

Higher field strengths became commonplace. Implants became increasingly sophisticated. Cardiac devices once considered absolute contraindications gradually became MR Conditional. New monitoring equipment entered the MRI environment. Remote scanning emerged. Point-of-care MRI became a reality.

Each advancement expanded access to MRI, but also introduced new questions that required thoughtful safety processes rather than simple yes-or-no decisions.

The ACR guidance evolved alongside these changes, expanding recommendations related to burn prevention, implant management, patient attire, ferromagnetic detection systems, emergency preparedness, hearing protection, renal screening, device labeling, and workflow standardization.

And with every update came additional responsibilities for MR teams.

Screening Evolved from a Form into a Risk Assessment

One of the clearest examples of MRI safety's evolution is patient screening.

Early screening forms focused largely on identifying whether a patient had metal in their body. Modern screening is much more comprehensive.

Today's screening process evaluates implanted medical devices, previous surgeries, medication patches, tattoos and body piercings, occupational exposure to metal, pregnancy considerations, claustrophobia, renal function, thermoregulation risks, removable objects, and previous MRI experiences.

The goal is no longer simply identifying whether an implant exists. The goal is determining whether an MRI can be performed safely under the specific conditions presented by each patient.

Patient screening became more than a screening form. It became one of the most important clinical decision points in the MRI process.

MRI Safety Became a Team Sport

As MRI safety matured, it became increasingly clear that no single individual possesses all the expertise necessary to manage every aspect of MRI safety.

This recognition led to one of the profession's most important developments.

In 2015, the American Board of Magnetic Resonance Safety, now the International Board of Magnetic Resonance Safety, introduced formal credentialing for MRI safety leadership roles.

Soon afterward, the International Consensus document established the collaborative framework involving the MR Medical Director (MRMD), MR Safety Officer (MRSO), and MR Safety Expert (MRSE).

Rather than placing responsibility on one person, MRI safety became a coordinated effort involving clinical, operational, and technical expertise.

Guidelines alone were no longer enough. Facilities needed defined roles, standardized processes, and multidisciplinary collaboration to build strong MRI safety programs.

The MR Technologist's Role Has Fundamentally Changed

Perhaps the most important lesson from the past 25 years is that MR technologists are being asked to do far more than they were in the early 2000s.

Today's MR technologist may be responsible for comprehensive patient screening, implant and device evaluation, coordinating multidisciplinary care, implementing MRI safety policies, participating in policy development, performing Full Stop Final Checks, managing increasingly complex patient populations, supporting emergency preparedness, collaborating with physicians, physicists, nurses, and anesthesia teams, and documenting MRI safety work.

At the same time, MR departments are often experiencing higher patient volumes, staffing shortages, more complex implants, faster scan protocols, remote scanning environments, and greater documentation expectations.

Practicing safely in MR today requires far more than technical expertise. It requires clinical judgment, collaboration, communication, and a commitment to continuous learning.

Documentation Is Finally Receiving the Recognition It Deserves

For years, MR professionals performed extensive safety work that often went undocumented or unrecognized.

The introduction of MRI Safety CPT® codes in 2025 represented an important milestone.

Rather than creating new responsibilities, these codes acknowledged work MRI teams had already been performing, including implant evaluations, risk-benefit assessments, exam customization, device preparation, and implant positioning and immobilization.

In many ways, these codes recognize the clinical expertise that has long been an essential part of delivering safe care.

Looking Ahead: The Future of MRI Safety

MRI safety continues to evolve.

The latest ACR guidance reflects growing attention to remote MR operations, point-of-care MRI, mobile MRI systems, increasingly complex implants, expanded RF exposure guidance, new staffing recommendations, and risk assessment frameworks for uncertain implant conditions.

Meanwhile, the introduction of the MR Safety Tech (MRST) credential recognizes that standardized safety competency at the point of care will become increasingly important as MRI practice continues to evolve.

Technology will continue to change. Implants will continue to become more sophisticated. Clinical workflows will continue to evolve. And MRI safety must evolve alongside them.

Supporting the Next Generation of MRI Safety

One of the clearest themes throughout the history of MRI safety is that safe imaging depends on more than individual expertise.

It requires systems that support consistency: current policies and procedures, standardized workflows, reliable implant information, clear documentation, collaborative decision-making, and technology that helps MR teams put established safety principles into practice every day.

The future of MRI safety will not be defined solely by stronger magnets or more advanced imaging technology. It will be shaped by how effectively healthcare organizations support the professionals responsible for delivering safe care.

Final Thoughts

Over the past 25 years, MRI safety has evolved from responding to individual hazards to building comprehensive systems that promote consistency, accountability, and patient safety.

Along the way, the role of the MR technologist has transformed from image acquisition to safety leadership.

And as MRI continues to advance, one thing remains constant: safe patient care depends on informed professionals, standardized processes, and a commitment to continuous improvement.

If the past 25 years have shown us anything, it's that MR professionals are committed to continuous learning, collaboration, and improvement. And as our profession continues to evolve, so will MRI safety.
